Insurance Information

We are committed to providing exceptional care and support to our clients, and we appreciate your understanding of our billing practices.  While we understand that insurance coverage is an important consideration, the ultimate consideration is an assessment that is able to access all of your needs, without limitation from outside sources. 

Dr. Eichenstein is not in-network with any insurance company. We request that payment be made in full during testing or consultation sessions. A superbill will be generated for you at the end of the testing sessions, which you can send to your insurance company or share with your private insurance advocate.  

We acknowledge that navigating insurance plans can be complex. It is highly recommended that you consider seeking the guidance of an insurance advocate prior to testing. We can provide you with a referral to an insurance advocate who can offer expert support in navigating your insurance coverage.

We encourage you to consult your specific insurance plan to understand their reimbursement policies, which often depend on your child’s diagnosis. It is important to note that insurance companies’ reimbursement rates for “usual and customary” fees are not based on actual charges.

For insurance inquiries, some CPT codes commonly associated with Neuropsychological Assessment include those listed below. Your final superbill may include additional codes as appropriate:

Office Policy

All fees are payable to this office directly at the time of the evaluation.  This office will provide a statement for your insurance company at the end of the evaluation. We will not directly bill your insurance company. You are responsible for all billing and collection of reimbursements from your insurance company.

Reschedule Policy

If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session as booked.  The only exception is sick children, where this office will happily allow you to reschedule at the last minute! 

Payment

There are several ways to pay.  You will be sent an invoice for payment, from which you can pay directly.  Or: please utilize the “Make a Payment” link for testing deposits and testing payments.  Cash and checks are also accepted.

Note: Unless there is a request in advance of testing, all fees are due in full by the end of the testing period.  Upon request, we can split payment over three months if we have your credit card on file.
